Express Scripts, Inc (ESI)

The Destiny Health Plan has selected Express Scripts, a leading pharmacy benefits manager, to provide you with comprehensive and cost-effective pharmacy services. Express Scripts' online tools can help you manage your prescriptions, look up drug coverage details, locate a pharmacy, or check drug-to-drug interactions. Members of The Destiny Health Plan can access these tools by visiting Express Scripts' website at www.express-scripts.com. Your prescription copay amount depends on use of generic, preferred brand or non-preferred brand drugs. Some benefit plans also feature lower copay amounts when you take advantage of Express Scripts' mail order pharmacy. Refer to The Destiny Health Empowerment Plan's classification of Generic, Preferred (Formulary) and Non-Preferred (non-Formulary) here. Please use this key as a reference when researching medications to determine the tier and copay amount. The specific copay amounts for your plan are listed on your Benefits-at-a-Glance.

Step Therapy

Step Therapy is a program especially for people who take prescription drugs regularly to treat an ongoing medical condition, such as arthritis, asthma or high blood pressure. The program is a well-established approach to getting you the prescription drugs you need, with safety, cost and-most importantly-your health in mind. If you have one of the conditions covered under Step Therapy, you may be required to try an over-the-counter alternative or lower tier drug before a tier 2 or tier 3 brand drug. Drug Quantity Management

To help make the use of prescription drugs safer and more affordable, our plan includes a Drug Quantity Management program. For medicines under this program, you may receive an amount to last you a certain number of days. For instance, the program could provide a maximum of 30 pills for a medicine you take once a day. This gives you the right amount to take the daily dose considered safe and effective, according to the latest scientific evidence. Based upon guidelines developed by the U.S. Food & Drug Administration (FDA), medical researchers and drug manufacturers, our plan developed this program together with Express Scripts. Specialty Medications

Express Scripts' specialty pharmacy, CuraScript, offers a convenient way to obtain your injectable drugs and supplies. Your specialty medications can be delivered to your home, your doctor's office or another approved location. You will also have access to other services available through CuraScript, including:

-  Access to experienced specialty healthcare experts

-  Guidance in how to take your medications correctly

-  Support in managing your medical condition

-  Personal care and health advocacy through a patient care coordinator

-  Complimentary medical supplies-such as syringes, needles and alcohol swabs

Specialty medications can be obtained on a once-monthly basis only

Chronic Medication Rider (CMR)

An advantage of The Destiny Health Plan, when funded with a Health Reimbursement Arrangement, is the option to add the Chronic Medication Rider (CMR). The CMR is designed to provide additional benefits to cover the cost of many prescription medications that are primarily used on a chronic basis.

Preventive Medication Rider (PMR)

An advantage to The Destiny Health Plan when funded with a Health Savings Account, is the employee's option to add the PMR. This rider provides first-dollar coverage, after payment of a copay for ten common illnesses that require preventative medications.
